What do I do if I paid more?

Fill out a Public Service Commission Form with the following rules:

Type or print in black ink.
You may submit additional information on 81/2 × 11″ paper, attached to PSC Form No. 1.
Do not write on the back of PSC Form No. 1 or on the back of any attached sheets.
The PSC Form No. 1 must be signed by each customer.
Each customer’s signature must be notarized by a Notary Public.
