This the proxy form that can be filled out prior to the Special Meeting. It is best to have this form filled out and submitted to a location on the form.

We know sometimes life pops up last minute, if you are unsure that you will be able to attend the meeting, or want to MAKE sure your presence and VOTE be counted, fill out and turn in a Proxy Paper. If you are able to attend the meeting, the proxy can be discarded!
